Well, if you care for a little HINT (SPOILER) then I could obviously tell you the simple answer.^^ I dont know if it was mentioned in the earlier episodes (I missed it then, and im pretty sure I watched all episodes at least once^^( but in one of the newest episodes (was it 163? Nvm...) One of the then current enemies disguises himself as the captain Rukia killed. During the fight he explains to her how he could disguise himself so perfectly that even she was fooled. Turns out she killed him because a hollow with the ability to take over the victims body managed to sneak up on the captain and attacked her in his body. She took responsibility because it was her sword that did not only not manage to save him, but ended his life instead. Quite sad Oo

Elfen Lied is bloody and gory, yes I agree. But Elfen Lied starts just as that with so much blood shed and head rolling, Its expected to see blood and gore through the anime, where School days start out as a normal romance school anime. School Days has such a bigger TWIST since the viewer is not expecting the things that transpired towards the end of the anime. Making School Days still the most twisted anime Ive seen. Also notice I said, school days resembling real life in my comment? How in the world can Elfen Lied be remotely realistic like school days?
